Knights of Columbus Gift Card Program

What is it?

A way for each of us to make a no-cost contribution to our church.
5% of each Gift Card stays in the program.
 
What does it do?
Knights sell Gift Cards from Food City and Krogers
for parishioners to purchase food, gas and pharmaceutical Items
 
Where does it go?
50% of all realized moneys from this program are directed
to the support of Our Lady of Fatima Catholic Church.
 
Form Help

Identifying Yourself
To order Knights of Columbus Food Coupons or Gift Certificates you need to identify yourself and establish lines of communication.  This part is done by filling in the two green Name boxes, the Phone Number box and the Email address box.

Navigating the Form
You move from box to box in order by pressing the TAB key on your keyboard or by using your mouse to place the cursor in the desired green box.
The order form opens with the cursor already placed in the first green box.
In the two name boxes enter only letters. The name boxes do not accept numbers, punctuation or spaces.

The green phone number box accepts a ten digit number only. 
The form provides the brackets and the dash so just enter your ten digit phone number like this "8659832027". The form automatically formats the number. If you make a mistake just highlight the entry and press "Delete"

The green Email box requires a valid Email address in the form "Joe56@someisp.net"

Placing Your Order
Now place your order(s) in the green box(es) in the QUANTITY column. No need to fill in unused boxes.
The COST amount will appear in the pink COST column automatically and will be totaled each time you make a Quantity entry  or change one.
It is not required to access the money boxes and if you should do so by mistake, no harm is done. You will just get an error message asking you to move the cursor to one of the Quantity boxes.

When you have filled up the top four green "identity" boxes and at least one Quantity box all that remains to place your order is to click on the "Submit Order" button. You will immediately receive an acknowledgement to let you know that your order was transmitted and received. If you happen to leave out a necessary item or make a mistake you will be guided to make the appropriate correction.

Starting Again
If you feel that the form is so messed up that you would like a fresh start then press the "Start Again" button. The form is cleared out of all entries and you are free to try again.
